Details make perfection… Ultrasound Imaging Biomarkers of Rheumatic Diseases

The main purpose of this presentation is to focus attention on the potential of ultrasound to show the presence of otherwise undetectable details that can play a key diagnostic role. The sensitivity of sonography is impressive: it is so high that it is possible to recognize minimal lesions such as bone erosions of less than 0.1 mm in diameter. Even if "sensitivity" and "details" are bad words for epidemiologists, as a clinical rheumatologist I like to remember this wonderful statement by Leonardo Da Vinci: "Details make perfection".

Ultrasonography (US) is a powerful and useful tool for the management of rheumatoid arthritis (RA). The first part of this webinar is about, after scanning standardisations, the basic lesions to look for: synovitis, tenosynovitis, erosions, where and when to look for them. Should we use US for diagnosis, disease follow up or/and remission? In the second part, we will consider the literature data to locate the interest of the US examination during the remission. Should we use US to reduce or stop treatment? Are there any data on the risk of relapse? To finish with, we will discuss the French recommendations, which should be published soon, concerning the use of US in RA.

Enthesitis is a common symptom of patients with spondyloarthritis especially with psoriatic arthritis. The clinical detection is most difficult in some cases. An important differential diagnosis is fibromyalgia. This lesson presents the ultrasound definition of enthesitis. Typical examples of enthesitis in inflammatory rheumatic disease were shown.
